Please click Here to invoke the annual report application.  Once you finalize your annual report in this application you will still be able to view it, print it, and print the affidavit but you will not be able to change it.  If you need to make changes after finalizing your annual report you will need to contact the Consumer Credit Section at (312) 814-5145.

The final Annual Report must be submitted by March 1.

A FINE OF $25.00/DAY MAY BE ASSESSED FOR LATE REPORTS
